Diffuse esophageal spasm: transforming into achalasia.

چکیده
Of 130 patients diagnosed to have esophageal motility disorders, transformation of diffuse esophageal spasm (DES) into achalasia was identified in one patient. The clinical syndmme of DES was first described by Hamilton Osgood in 1889. It is characterizedby symptoms of substernal chest pain and dysphagia, tertiary contractions on barium swallow and a manometric pattern of frequent simultaneous contractions, interspersed with normal peristalsis. It accounts for 4-15% of the esophageal motility disorders identified in patients with non-cardiac chest pain. Various lower esophageal sphincter (LES) abnormalities i.e., incomplete sphincter relaxation, hypertensive LES have been reported to coexist with diffuse esophageal spasm. This suggests that some of these disorders may represent a dynamic spectmm of esophageal dysmotility rarely, transforming one into the other condition. This report describes one case transforming from DES to achalasia.

We report the occurrence of achalasia and diffuse oesophageal spasm in brother and sister. The familial occurrence of these unusual disorders is consistent with the possibility that they share a common aetiology and indicates the potential importance of genetic predisposition.

Diffuse esophageal spasm is a rare motility disorder. The primary symptoms include dysphagia, and chest pain. There is array of treatments available from medical therapy with nitrates to surgical therapy; Heller myotomy. We present a case of severe Diffuse Esophageal Spasm (DES) that showed one of the highest Distal Contractile Integral (DCI) per manometry. Patient was treated with Botulinum To...
متن کاملIneffective Esophageal Motility Progressing into Distal Esophageal Spasm and Then Type III Achalasia
The clinical significance of minor esophageal motility disorders is unclear, though they typically carry a benign course. Distal esophageal spasm progressing to achalasia has been reported, although it appears to be rare. We report a case of a patient with dysphagia and chest pain who was found to have ineffective esophageal motility on high-resolution manometry, which developed into distal eso...
متن کاملReinterpretation of Follow-Up, High-Resolution Manometry for Esophageal Motility Disorders Based on the Updated Chicago Classification
The aim of this study was to assess changes between primary classification of esophageal motility disease and follow-up classification by high resolution manometry (HRM) and to determine whether previously classified diseases could be recategorized according to the updated Chicago Classification published in 2011. We reviewed individual medical records and HRM findings twice for each of 13 subj...
